Ishii, who is undergoing anti-cancer treatment, has had his cancer metastsize to his liver, lungs, lymph nodes, etc. He decides he wants to switch to a treatment that prioritizes taking away his pain. Ryuji is saddened by learning he will no longer be treating his condition. When the two are outside talking, Ishii vomits and has to have surgery.
