My headcanon for this is partly influenced by some comments that I read from other viewers who read the novel that this drama is based on.
My interpretation is that the leads had a wedding ceremony in the historical novel world and lived relatively long, happy lives in that world together. Then the ML died first in that world and returned to his life in the real world as a teenager in 2016. I imagine it was a bit of a rough adjustment at first, after living in the historical story world for so long, having to live as a teenager with zero authority after living as an adult Emperor for years in the historical story world, and also having to be away from the FL for almost 10 years (between 2016 and the end of 2025 in the real world), but I like to think that it was also a very positive experience for him to be reunited with his parents and to get to have a relatively normal teen life (after the awful one he had in the historical story world). Plus he would be able to use information supplied to him by the FL about things that changed between 2016 and 2025 to make some very lucrative investments.
It maybe would've been difficult for the ML to stop himself from approaching the FL in the real world until after she experienced everything in the historical story world, but he found her, observed her from a distance, and made sure that he was in the right place and the right time to get on the correct train in the evening before her birthday in 2025 so that he could be there waiting for her the second she returned from the historical world. He probably also made sure that his life was arranged so that he already lived and worked near where the FL lived and worked in 2025.
From some comments from other viewers who read the novel, I gather that the ML turns out to be the CEO of the FL's company and the person who bought the rights to adapt the transmigration novel into a drama, but I personally am not a fan of that (it creates a paradox, I think it's too unrealistic for someone in their 20s to be the CEO of a large drama production company, and I prefer the idea of the ML having a more fun and relaxed job / life in the real world), so I exclude it from my headcanon. I prefer the idea of him being an artist who does things like set design / prop design and sometimes works with the FL's company. And in particular he specializes in set design / prop design for historical dramas. That idea is inspired by the scenes we saw of the sketches he drew after arriving in the historical story world, and I like to think that it would also allow him to draw on some of the knowledge he had from the historical story world about how historical buildings / palaces would look / work on a practical level.
For the FL, my headcanon is that she served as regent for a few years after the ML died in the historical story world, and then (after she transitioned power to the next Emperor and the kingdom was stable) she also died in the historical story world and returned to the point when she left the real world in 2025. And, as we saw in the drama, the ML was ready and waiting for her there and they both retained all of their memories from the historical story world. So they resumed their relationship and married again in the real world.
