in the pilot trailer they stabbed his private part. if he were to try and fixed it at a doctor it would be hard they would either have to stitch it back on or a transplant. getting the surgery to change it to a female private part would be easier since they only use some of the tissue from the private part. also he can't just keep his appearance at walk out in the streets like normal because they made him fake his death
