Since OG Tin isn't available in those 3 days. Lots of things could change, also keep it in mind that Triage did something different from other loop dramas.
From ep 1 Tin can go back further into time as he wants to, which has never been done in any loop drama

About the changes. It was just minor changes. We have Tin returning back to who he was b4 the loop and Gapsing Dating
I think the reason it's different is cos triage took a step that's different from other time loop movies we've seen
Like in Groundhog Day and the rest, the people stuck in the loop can't go further than the time the loop started. But in triage Tin is able to go back as far as he wants (I think that's what make triage special and unique from other loop dramas)
Back to it. Here in triage whenever Tin go back, the previous timeline reset and the current one is now the permanent one. The last loop b4 the switch, Tin went back to a week b4 Tol's death and After he die. Jinta took Tol back to July 14th the day Tol became aware of the loop.
And with Tin having no memories, time has to heal itself. That's why Sing and Gap figured what Sak was doing. And since they've been dropping crumbs since episode 1 they probably got together since Tin wasn't there.

In the og timeline, there was no Dr dear but because Tin went back a week b4. He encountered her Even the man who stabbed Tin wasn't part of the original timeline
Lemme give you an example of time fixing itself
B4 the loop, Tol was the one who hurt Mai not Hart. The party originally was supposed to be Mai's birthday party but since Tol and Mai broke up... Another party had to happen in it's place. In July 18 10:55 Tol was the one who died but instead Tin died in his place.

Abt the buildup. You know it's a time loop. Whenever the time reset, everything starts from point one and apart from the ones aware, every other character is back to how they were
Quality of the drama went down when tol loop started.
I highly doubt that tho. For me the quality never dropped. And I enjoyed Tol's loop alot. They got to be together We saw Tin's love b4 the loop Gapsing got together Mai's mom survived and lots more
