This has been mentioned before, but these effects (tattoos and his eye colour changing) are only used to make it easier for the viewers to know when his personalities are taking over and when they're present. He needs a psychiatrist because he is suffering from D.I.D.

Does this drama have any romance in it?
Among the dramas I'm watching, Spy is definitely one I'm enjoying, but I wouldn't say the plot is strong enough. There are moments here and there that are suspenseful, but overall the drama isn't as fast-paced as it might seem to be.

There are still a lot of unresolved conflicts so if they do it right, it can turn out to be a good decision.
