Oh you are interpreting it all wrong. The dentist was killed by him and also he killed others. The dentist wanted to make him like all of them that's why he says you are the best piece of my work or somewhere along the lines. And when he did actually end up killing them all so despite the dentist dying, the dentist still won because he succeeded in making him like all of them. He made him a psychopath by triggering his PTSD time and again. That is why we keep on seeing the glimpses of the dentist in his face in the end because despite the dentist dying, the dentist is still going to live through his piece of work aka the ML.
