I think the doctor guy and his sister are maternal cousin. Xiangxiang amd xiaoman's mother's nephew and niece. Because I have noticed in historical cdrama they don't call Father's side cousin as cousin. They call them brother or sister. And this drama is set in the middle of ancient and modern China. I think at that time they still followed that rule

The drama's original episode number was more than 40. Because they adapted all 4 books and FL's total screentime on the drama was supposed to be as long as 37 episodes and they filmed all. Then there are ml & other supporting characters scene. So it was definitely not 40 eps drama. But the yin-yang script thing did happen. They wrote another script for ml and wanted to add those in the drama while removing fl's already shot scene.
