Well, the only truly worst outcome of sibilings sexual relationships is genetic illnesses, but this happens even without being a close family, so since two men cannot biologically have kids with each other, when we forgot about some human-only reasons there is nothing against it. I know in some countries romantic and sexual relantionships between relatives are forbidden by law, even when it's someone not connected by blood.

Well, it's been so long since I started being in Japanese world and it was weird for me in the beginning too. But calling ones lover by their last name is nothing compared to wifes traditionally calling their husbands "Danna-sama", which literally means "My Lord". By the way, what's interesting is that while Japanese custom put family name first, then their last name is their given name, and their first name is their family name. So in Japanese "Takase Ryo", "Takase" is his last name, but it should be called "surname", as it's paternal last name, coming before given name "Ryo", as a name that his parents gave him. It is common in Asia, but in Europe or US it's often mistaken.

The fact is that calling lovers by their last name in Japan it's not only cultural thing, but an individual one too. There are many cases when someone call his lover by his last name for years, especially when is two men, and waiting for a right moment. It's something like "another base". ☺️ But then, Japanese are very slow when it comes to it, and they are very careful to show affection in any way.
