I think he has an underlying condition that hasn't been revealed yet. Though I wouldn't be surprised if it turns out he also has some sort of heart disease which is similar to his older brother and is hereditary.
I honestly don't understand the hate this drama is getting. It's honestly a good thing they brought the cat out of the bag this early rather than push that marriage contract revelation in last episode like most contract marriage kdramas do.
