Reason why she's still pretending around him is she doesn't know he knew she (FL) is faking the illness at this moment, the FL also doesn't know if ML is friend or foe, so she has to be very careful as she's a hostage and a target. Better be safe than sorry I guess

hmmmm, the visible difference is pretty obvious, and, the ML looks younger than his actual age while it's the other way round for the FL
