It's mostly subjective. Majority don't like Makjang so viewers down voted. Plus the sensitive elements of Infidelity.
The Only Makjang Drama with Infidelity theme that was rated high here on MDL was TWOTM at 8.4. Maybe because it was an Adaptation of the UK series "Dr. Foster".
Most of the Makjang drama recommendations here are rated below 8.
"I have a Lover" is rated 7.8 considering it might have a better plot than Red Balloon.
It must be the Genre of Infidelity/Makjang.

I wonder why KBS decided not to pick-up this drama when Moon Young Nam's last dramas "Revolutionary" Sisters and "Liver or Die" were shown on KBS.
KBS decided to pick-up "Three Bold Siblings" instead which has the same timeslot as RB and is more wholesome than RB. Plus I am sure RB will get R-18 rating if shown on KBS.
If RB was shown on KBS, it will have double digit Nielsen ratings like RS and LOD because it would have a wider viewership exposure vs Cable(TV Chosun)

This is true. Especially for Makjang. Happy Endings are usually for Romcoms

Most Makjang dramas don’t have good ending
Most of it are open ended i.e. TWOTM or Nice Guy
