let me rephrase i get that Moon Cha Young(Female Lead)'s Brother knows Lee Kang(Male lead) as his sister's First love but how he knows that Lee Kang(Male lead) 's First Love was also Moon Cha Young(Female Lead).

i think this is either a plot hole or FL brother just bluffed to push away the nurse.

There was a moment when Moon Cha Young(Female Lead)'s Brother tells the Nurse not to fall in love with Lee Kang(Male lead) cause that Lee Kang(Male lead is the first love of Moon Cha Young(Female Lead), Then corrects himself that they are each others First Loves. How did he know that??? i cant wrap my head around it!!!.
