His mouth got sewn shut so now he eats thru a tube. He tried to make the new delivery driver be quiet to not alert the woman who cut pieces of herself off to appeal to dong in. He is now trapped and will live the rest of his life as her pet.

It goes back to the fairy story at the beginning of the episode where the mother says everyone has a fairy in their hearts and that they do not disappear until someone else tells them that they don't exist. The mother believes that her daughter (the fairy) still exists hence why she is sees her as a hallucination. However when she is told that her daughter is already gone that hallucination disappears. Everything regarding her daughter is her own delusion.
