Whenever this has happened to me, it's because they were already on the list. Most often because I'd already added them and forgotten. But also a couple of times because the site glitched momentarily - they were added to the list but not showing yet.
If they're in the database, the most likely reason is they're on your list already.
(Apologies if you've already figured this out. It might help someone else who's run into the same problem. It's easy to do.)
