Yep, was part of the husband's plan to downplay his health condition, so the doctors won't do so many checks, and in turn both save money and able to push blame onto doctors for performing surgery leading to his death.
So the wife continued with her plot, and kept insisting it's the doctors' fault. Until later, when her daughter convinced her of her folly that she repented and tried to make up for it.
After reading through the comments and rewatching the part, I reckon the ML had lost his life too.
The reunion scene with the others who had, in one way or another, made sacrifice in their line of duty. With the ML are only the three people - the former "battalion director" lost a leg, the team 2IC (engineer-convert) and early-discharged fighter who also lost their lives in an attempt to perform their duty in/out of uniform. With the former battalion director amidst the audience, it just proves the reunion is that of people who have made sacrifices.
Granted that the ML might be on duty in a distant area as some might have pointed out (which is also a sacrifice), the later part where he enters the hall and subsequently only his claps were audible only to the FL... leads me to think the sacrifice the ML took is his life.
Given where the film is from, it's more of drama to showcase the sacrifices of the firefighters > love arc of the couple, hence they left it as this "open ending". Wish it wasn't like that, just him sitting amongst the audience would be fine (dont even need any wedding scene) but oh well...

Maybe there were bts unhappiness that the actors just kept mum - and we'd never know - and it's abit ridiculous why they blaming GYJ instead of the directors/producers.
