its just the unrealistic beauty standards of Korea, and she is portrayed as "buffed" bc she's a weightlifter, (even tho the actress is really skinny) even on her first visit to the clinic the doctor says shes even healthier than he thought she was but she has too much muscle. I think they're just "plot holes" but the story itself it's good
