I have a theory, which I would like to share with you all.
In the preview, Kwangsook was holding an old photo of a young couple. She said to herself, "How did she get this photo? Who is this man?" That means Kwangsook recognised the young woman (presumably her grandmother) but not the young man.
Jusil (Kwangsook's mother) and MIL are probably half sisters. The young woman, in the old photo, could be Kwangsook's grandmother who was both MIL's and Jusil's mother. The young man must be MIL's biological father.
During a natural disaster ... typhoon or earthquake, the young man was carrying MIL and somehow both got separated from the young woman forever. The young man died of his injuries so MIL was sent to an orphanage where she was adopted to the US. Young woman remarried and Jusil was born.
Do you think this theory is possible? Or Kwangsook is MIL's daughter? It is weird to have two daughters marry the same man... to me

MIL was extremely arrogant and disrespectful in her conversation with Kwangsook when she returned her medicine in Dongseok's house. But the Writer made Kwangsook's character sit quietly and swallow all the insults thrown at her. This is frustrating to watch.
Having said that, chingu, it is a fact that dementia patients do have a change of character especially becoming impatient and angry easily. I have read about it and a friend's MIL is a dementia patient.
