Third brother means there was a 1st (eldest) and second brother. It wouldn’t have anything to do with a sister or sisters. Either way, in this drama, they don’t exist meaning they died already or somewhere else.

They do give us a hint or the characters talk about it but it’s really quick and can easily be overlooked unless you are 100% paying attention. It doesn’t help the subtitles aren’t the best either.
