I was also hesitant on watching but decided to give it a try, I am currently on episode 10 and I'm enjoying the show a lot. To answer your question: 1) the swap was unintentional, they got into an accodent and swapped. The grandpa tried to give his grandson his body back but since he couldn't he decided to live well for his grandson until his spirit comes back. The grandpa is a really kind guy.
2) He kind of saw his wife in Hai Yuan but I think it's because he is the reincarnation of his wife, he was his wife in the past life. The grandpa and Hai Yuan are soulmates.
3) Im not yet at the ending but all I know is the grandson is dead but his spirit comes back some times to just observe.
All in all I recommend watching, despite the very peculiar plot, the romance is very sweet. There's some unexpected twists and also kinda cringe but I like it. It reminds me of old school bl.
