That part I think I get. She did stab herself to kill them both. Hwal already had a fatally injured heart but since he wasn't dead yet, neither was she. He turned OET into a bulgasal right before he died. This way, she survived as well. Plan was for OET to eventually turn him back into a bulgasal but she did it first.
