I didn't watch it completely but from the clips I saw, it's probably not a cliffhanger (but there's a clip about the last episode which makes me confused if my prediction is correct)

It's the opposite --- the entire story was his consciousness and the ending was the real one

I guess Eun Hyeok got infected as well (it felt like the blood in the ending was nosebleed) so probably he survived by the Golden Hour (the ending of the story definitely requires a new season and as he's one of the main characters, I don't think the writers would make him die so easily)
