I disagree with you, the first episode was executed without rush and confusion and it EFFECTIVELY introduced the character and his life. Some of the crime dramas usually have first episodes with a lot of mystery which is good, but lacking clarity and understanding for the audience. The purpose of those "10mins" scenes in surgery is to contrast both sides of what the Main plot is. Which is that although he is a surgeon he has another side that is unethical however is still a doctor by saving peoples lives regardless of whom they may be. But don't worry, this dramas has potential, which why first Ep is chilled ;) usually plots will engage us more as episodes go by and we just have to wait and see. I say these things because joo won is a great actor and makes the whole drama a whole lot better.
