I feel your pain. Lack of communication is one of those devices K-drama writers use to frustrate the heck out of viewers. Keeping relevant information from others that should know the information creates even more drama when the information is finally found out in the worst possible way. There is always a justification for keeping secrets.
The doctor seems to want to keep Jae In close, and neither he nor the uncle is explaining anything to DoHee's mom. So, of course, she has no idea why those 2 are acting the way they are. DoHee's mom will eventually find out anyway.

I really hope the aunt does not get a redemption arc. I know K-dramas love to either redeem terrible characters or allow them to escape any form of punishment.
However, she took complete advantage of her late brother's kindness, screwed over his wife, abandoned her son, is still lying to him even though he is a grown man now, and now has abandoned her niece who never did anything to her.
I hope her recent conduct is exposed to the board for backing an adulterer, thief and murderer for chairman, and she loses her position in the company. I don't think Jae In needs her help nor do I think Jae In should trust her.
