Understanding a language and and speaking it are two different things. You can understand but struggle to actually speak or do sign-language. So its totally understandable that he's been understanding all along but just isn't fluent in speaking it.

It's actually very clear that he does understand sign language, it's just that he can't speak with it very well. He literally told Hejoo I already know the alphabet I prepared before hand. He's learnt it since 20 years ago, but he's not gonna tell her that. But since he hasn't spoken with it he can't speak with it but understands and he's understood all along.

I think it's because he directly say's what he feels and wants to Hein, while Hyunwoo doesn't communicate his feeling to Hein..... If he only communicated his actual feelings they would've made up by now, while ES says it directly to her face.
