IMHO, I think the King really loves the Queen. The way he hesitated to kill her many times and his facial expression when she's having "heart attack" speaks volume.
About the flashbacks, we can only see bits and pieces of it and not the whole story behind it. I feel that the writers wanted us to believe that he really hates her so much. It's probably more of trust issues because she belongs to the Kim clan so he can't love her 100%. King Cheol Jong's eyes betrays him in some instances.
Also, I noticed that the king was trying to always have a "skinship" with her, the wedding night scene when he held her in his arms and the scene when he woke up after the nightmare, he's still holding the Queen's hand and doesn't wanna let go. lmaooo. But let's see... :)
