Why are people still talking about a mental illness when there is clear proof that the twin does exist (she even knows about him) and is clearly trying to drive her crazy. At this time in the story it is too early to draw any conclusions let the drama flow . I am sure it will all come together in the end. Actually I don't think he cheated.. they did go to a hotel room but the girl was really drunk so probably threw up and had to change clothes and then just fell asleep with the bathrobe on.
