Until I have seen episode 6 I was convinced that they are both dead. Wearing the same dresses which they wore at the time of the accident, etc. I have even commented. Not so sure anymore. It was not that apparent to me for the first watch, but Monica is not talking to Ken there. She is talking to herself. Ken is just there. And she probably neither sees nor hears him. And Ken wears the same t-shirt there too he did at the time of the car accident, yet without the shirt on.
Since we have three more episodes to come, I still give the benefit of doubt that Ken is possibly in the state of clinical death/coma only. But who knows what is in the screenwriter's mind.
