this is my interpretation. there are two options from this open ending.
option 1: the surgery causes jong bum to be in an almost amnesic state as seen in epilogue. sa bin helps him remember and he finally does. they are both seen in the bokshop looking at eachother. then blah blah su hyuk comes with flowers and realises.
option 2: the surgery fails, jong bum loses his memories and eventually dies. giving his heart to sabin as we see she feels "healthier". she goes into the book shop and remembers jong bum and almost sees him, because remember she may have her heart so it feels like he is with her. su hyuk comes in with flowers and i guess tries to remend their relationship. this could be the case as you so the gang leader guy hug su hyuk about his brother's retrial and smile at su hyuk when he talked about sa bin. it seems like he knows something is going on.
