No, this user literally said "I wish you or someone close to you will get a cancer/leukemia". No matter the context you shouldn't wish that to anyone ever. It's normal that people have different opinions and often argue because of that but there's a line that shouldn't be crossed yet that person did just that. What's worse she was aware that she's doing something bad since she mentioned that she usually doesn't wish this to anyone and yet she proceeded with her insults and when called out didn't even try to apologize. It was not a figure of speech, it was wishing someone to get ill because of someone not agreeing with her. That person did it quite a few times today; attacked, shamed or straight out insulted people for having different opinion than her. Typical behaviour of someone who thinks they can say whatever they want because they are anonymous in here.

Yeah, coughing blood was most likely for dramatic effect to let viewers know that DoHa caught the disease and is soon going to die soon. Could have done it better because the whole scene turned out being weird and unrealistic.

That's right but I guess they just wanted to let viewers know that DoHa caught the disease due to being exposed to radiation and this coughing blood was simply for dramatic effect. Not their best move to be honest because it turned out weird but there's nothing we can do about it. Would be much better though if ChangHoo dealed with him in Vincenzo style. His revenge on DoHa was not very satisfying considering all the things he has done.

Why should we read his eyes? Not every viewer notices small details like supposed pain in his eyes. Mostly viewers are able to connect with character's pain through their facial expressions and in this case there was little to none. Lee JongSuk is a great actor, he proved it many times that's why lack of emotions on his face during his character's confrontation with MiHo after finding out about her disease and especially during MiHo's death scene is even more confusing. I wasn't expecting ChangHoo to get all histerical during these scenes but his reactions were supposed to have us sobbing and mourning MiHo's death with him. Yet I felt absolutely nothing. And as I can see I'm not the only one feeling that way.

By squeezing everything I meant ChangHoo losing to DoHa, MiHo's death and eliminating all antagonists - it all happened in one episode while it should be divided into two eps.
Big Mouth's ending would be much better if it was divided into two episodes - Episode 16 where ChangHoo loses to DoHa + MiHo is suferring from her ilness and dying soon after, AND episode 17 where ChangHoo avenges his wife and all victims of radiation by eliminating all antagonists one by one. Instead they squeezed everything in one episode which made the ending rushed and not very satisfying. I said what I said.
Well, MiHo death was to be expected all things considered. It's not a secret that Big Mouth had it's plotholes but letting the character with 4th stage leukemia survive and be cured would bring this drama to a a soppy opera level. And that's obviously no good. So I'm not surprised nor disappointed with MiHo's death. It was clearly unavoidable. If there's something disappointing with MiHo's death it's the way it was delivered. Don't get me wrong, Lee JongSuk is a great actor but I wasn't really feeling his scenes with Yoona in the last episode. Maybe it's just me but it lacked something, it felt like through the scene in ChangHoo's previous office where he confronted (if you even can call it that) his wife about her disease, JongSuk's facial expressions were almost the same the whole time until the very last moment where he started to apologize to her for not being there for her. To be honest I'm surprised how ChangHoo seemed to accept her sickness and unavoidable death so quickly. It was like "you're sick? Oh no i'm sorry i wasn't there for you but i guess it can't be helped, now let's go back to my campaign" ... oh well.
And don't even get me started on Choi Doha. After all he has done, after becoming absolutely despicable, one of the most disgusting antagonist I've seen in a while (kudos to the great actor who played as him) all he's getting is this? Don't get me wrong, I like it that it's ChangHoo who killed him but the way he did was so... tame. I mean if you have seen Vincenzo then you know that kdramas can give an antagonist the ending he deserves but somehow there they just ended it in such a simple way it was not satisfying at all.
In general it was a good drama, a very good one if we are talking about episodes 1-14. But in Big Mouth happened something I was worried about the most - they screwed up and rushed the ending. After absolutely epic 14 episodes it started going downhill. But perhaps my expectations just were too high. Still the drama was good and I can honestly recommend this as a well done but perhaps a little rushed at the end thriller drama.
Perhaps MiHo illness is what's going to be a main factor in ChangHo's desperation in trying to find a cure and solve Doctor Seo Jae Yong's paper case as fast as possible. That's if MiHo will even tell him about her leukemia which I honestly doubt. Might be that she will try to find a cure on her own without disturbing her husband, who's completely absorbed in trying to bring down Choi DoHa. Or might be that she will eventually tell ChangHo about her illness but it will be already too late and MiHo will die. That would history repeat itself and make Park ChangHo's path to becoming a Big Mouse much more tragic. Personally I would prefer a happy ending that much more but with these writers and this show? I can't be sure of anything.
