Lots of BL, especially Manga, weren't mainly targeted at woman, just take some of the very first works like “Kaze to Ki no Uta” that got little approval from female readers but lots of high praises from critics. It's Shōjo only because it couldn't be marketed in any other way, BL always had lots of variety that was geared towards males and females, those works just aren't as popular because of the loud (toxic) fujoshis that are despised even by their own community. BL is like straight romance, overall more popular with women but not specifically targeted at women, it's just that most of the quality stuff gets overshadowed by whatever the loudest in the community favour.

As far as I can tell, most people call something “BL” when it's focused on the gay love, and “LGBT” when it's more focused on the plot. BL = gay romance, LGBT = story that happens to have a gay couple. In book publishing, BL is the same as LGBT because the latter means that the character's sexuality or gender is an essential part of the story, while “happens to have a gay couple” would be listed under the book's genre and not in the LGBT section. What the community is doing now is nonsensical, they try to split one thing into two just to distance themselves from the BL community that they think of as less sophisticated and serious.
