the marks on Na Il Deung neck were from his repetitive scratching of the skin due to his stress. Ri Eul back scar is from when he jumped off the building. It wasn't explicitly explain, but inferred. Also they didn't explain the butterfly necklace simply to leave a bit of mystery and magic. I would assume RiEul bought the necklace since the first thing he saw when he was stressed in high school was a blue butterfly.
