The movie kind of laid out everything that was going on, but basically Su-mi (the main girl) WAS the step mother…
She was the home nurse who had an affair with their dad which caused their sick mother to commit suicide and Sooyeon dying from being crushed by the cabinet trying to save their mom. I think the ghost who killed the stepmom was the wife and not Sooyeon. Their aunt even confirmed she saw a "girl' underneath the sink.
