For screen time i don't have justification. But the backstory which they have given to SML ,him acting like that is understandable as girl he likes is cohabiting with someone. Also I have seen dramas where SML's story as main plot that why it was easy for to watch him like that.

I don't think it is case of unethical. It is just that for patient might misunderstand care and compassion getting as romantic one. And for doctor there might be compromise on proper medical practice if there is deeper relationship.(BTW I have yet to see this drama)
