I mean that in the manga, Kainuma is a petty thief who Maki let off because he felt bad (he was shoplifting food iirc) and afterwards, he got an obsession with Maki and killed the 28 boys. In the drama, he isnt a petty criminal. He is a friend of Maki who helped him establish the MRI method of investigation as he is a neuroscience professor. His motive was that he had always loved Maki and had an obsession with him. In the manga, he explains he also hated Maki so he took it out on boys who looked like him. As for an explanation, I think its explained in the scene where Aoki is talking to Okabe and Okabe explains he is a sadistic killer and there isn’t any point in trying to understand him. My takeaway is he has a deranged mind and this was his way of showing his love for Maki. And since he has a deranged mind, he knew he would live in Maki's head by doing this act. That's my interpretation at least.

I watched the anime, manga and movie to prepare subtitling for this. I'd say the most significant change is that Suzuki was shown in flashbacks only in the anime and manga while in the drama, he worked on the Kinuko case with Maki. Maki also has Aoki's dialogue while Suzuki has Maki's (from the manga). Another significant change is Kainuma. He is not a criminal Maki knew but was Maki's friend and helped him develop the MRI technology as he is a neuroscience expert. He still murdered the 28 boys. Besides that there's only some name changes, like Takizawa is now Takimoto.

He didnt say he betrayed the man, he's talking about this after seeing happy images from the dog. I took it as a self reflection because this case didnt have a happy ending for anyone in the end. His pursuit for justice here didn't bring any positive change. The secret that Tsuyuguchi was meant to take to the grave was exposed, but nothing positive came from it - the boys all died, Hirai died, his dog died, Kinuko is arrested, Tsuyuguchi is still dead. Was it right to expose a secret that was meant to be left in the past and didn’t have a positive effect? Thats what hes grappling with. New sins are areas where he feels he crossed a moral line by uncovering or exposing something he shouldn't. New secrets are where he uncovers more secrets (both in Kinuko's case).
