If he is trans then there should be scars on his chest, did you notice that the camera angle in the shirtless was shot in such a way that you can't see that part of his chest. Maybe it is becsuse I haven't read the comic yet but I didn't find it rushed at all. Atleast there wasn't any weird time gaps, it was smooth and I liked it.

I think what she meant by miracle is that saen mentioned to her that the presence of his loved ones might help him recover faster or something like that. So I think she wants to use his ex's memory because that is his happiest moments to try to gety him to wake up. Like use a photo or something related to them.
