Medicines can prevent the heart decease. He's aware of it now, so he's taking drugs
Did he magically teleport with the meds in his pocket for those x days he's there and they just never addressed it or show him take it? I doubt that. Also, meds aren't 100% effective. From what I've seen, there are meds that can rapidly decrease/increase blood pressure if need be, but they would have to show him take it when he's having a seizure imo.
You could, of course, say that Jinta brought the medicine to him. Or cured him. Or that Tol somehow got diagnosed and prescribed the medicine inside the current loop and that it was enough to prevent him ever having a seizure, even though they never show it, either.
I still consider it a plot-hole, because it's important part of the story they just never addressed again.
One of my plot-hole examples that can't be explained away by the "it's an alternate reality / different loop" rationale is :

His heart gave up when he saw unimportant Mai leaving with Heart. How could he get through this hell and have no other seizure? Past 4 episodes were nothing but suffering for him. Is his heart made of steel now?!

EDIT: We can see a bottle of pills in episode 11 (30:34 mark). We don't know for sure if those pills are for his heart, but let's assume they are. How did he get them in this loop? At the start of the scene, there's a zoom-in on Tol lying in bed (27:07). The pills aren't there yet. During the scene, Tol lies in bed, has a dream/vision and boom! Pill bottle appeares, as if it were magic. The scene doesn't seem to be cut from his pov, so if Tol got up, grabbed the pills (that he acquired somewhere beforehand), then got back on the bed, he would have to get to (roughly) the same position and we just weren't shown that in the show. But my money is on magic.
This establishes that Tol might have access to his medications, and he also might have been taking them (though we never see him take a pill in the new loop afaik). Are those pills enough to get him through that hell of a loop without any incident? I would say, not likely. Tol also gets a seizure in second to last loop, some time after he's been diagnosed in it (so presumably already on pills). You could argue that he might have not taken the medication at that time (since we didn't see him take it). But that's pretty unlikely, since he's been with Tin that day, talking about his health and how nervous he felt about the evening event. I want to believe Tin would have made sure Tol had taken it in that situation. And if not, it sets a precedence of Tol just flat out not taking the medication, even at important times.
So, to sum up. Tol may have an access to his medication (are those his heart pills? How did they get there?), he might even be taking them (though we never see him do that), and those pills might be enough to prevent his attacks (even if we saw him have a seizure at a time he was presumably taking the pills). Honestly, that's too much "ifs" and "maybes" for me, none of them addressed in the show. His heart was still at graver risk than that of Tian from ATOTS imo π€

I agree about the last loop, though I think the ending was about as good as they could have made it with such a setup, if they wanted a happy ending. I will probably rewatch it again to see what I would have changed, but overall, it was fine ending imo.
About the last scene: I have no idea. Some people think a novel named "Euthanasia" will be adapted. Some others think it's "Transplant". "Make a Wish" is suppossed to come out early 2023, but I doubt it's related to this. So, we will just have to wait and see.
My first BL was 2gether and I'm perfectly satisfied with that. Good enough to make me look for a next one, not strong enough to give me unrealistic expectations. I might even rewatch it, some time in the far future, but solely for the nostalgia π
