I feel like knowing the story just ruins everything. It's one of those BLs that's better watched in cute edits on YouTube because the chemistry was great but everything else sucked.

No one really knows what happens in that state. This might be an interpretation by the author. Or it could be from the perspective of the main character. It's not necessarily realistic either, this might just be the way things work in that universe.

I think everything is explained what's needed. The world they came back to is real. The world they went to is the Borderland a place between life and death. Those who die, die. Those who win and chose to come back, go back. Those who stay, are in a coma. According to a quick google search in manga the Joker is a ferryman who carries the dead to the netherworld. Which only makes sense why we would "meet" him after they came back, they had to "take the ferry back". Losing memory also makes sense. They were in between. What else would we need to be explained?
