I don't think she is mean or arrogant... The character is just awkward and don't like to mingle with people... She tries to help her patients as much as she can by not giving them unwanted treatments and does a lot of charity even when she dont have enough money... A mean and arrogant person would cut off all those charity if he/she gets into the slightest of difficulties or won't do charity at all... It's clear from the first scene that she don't like to mingle with people and gets awkward around unfamiliar people like that old lady in the lift but she looked after that lady well and when the lady thanked her over the phone she was very moved... She clearly cares about her patients and that's why she got into a fight with her senior even though she had a motto of not getting involved in other people's business...
