It would be terrible writing if she was the mother.
She's clearly the mother, it would be weirder if she a was a sister the show never mentioned.

I think it's important to note:

1. MY's mother's book series was called "The Murder of the Wicked Witch of the West", and the Wicked Witch of the West in the classic WOZ books was known for having power because of the vast array of minions she controlled in the books (wolves, crows, flying monkeys), and she was allied with the Wicked Witch of the East (original owner of the silver/ruby slippers) who was killed when Dorothy's house fell on her.

2. Twice in the series, they reference how in ancient Greek the word for butterfly is "psyche" which means "soul/mind". However, the "butterfly" brooch in the series is two butterflies.

How? I never see that coming
I've known it since episode 3, it was obvious because:
1. The head nurse was given no story development or personal details despite having ample screen time and being present in many scenes. Even the smallest supporting roles have been given development. So from a logical standpoint she had to have a significant role that hadn't been revealed yet. Since she didn't offer any substance, it seemed to me as if the show was purposefully making her character notable but not suspicious. I've watched so many dramas and read so many books that I've learned when a show has a mystery plot like this I always suspect those characters first.
2. If we were supposed to assume the mother was still alive, it wouldn't make sense for her to be a patient when she liked having control.
3. The way the head nurse was so insistent to give her opinion reflected on the the controlling way that the mother was in flashbacks.
4. It just seemed like she was the only choice considering age and if the show wanted to surprise the audience.

I am going to be disappointed if they say that is the mom. The dad killed the mom and dumped her body. I think…
Yeah, but that could be the reason she had to get plastic surgery, her face could have been injured when the husband tried to murder her. A sister would be weird because I don't remember them mentioning the mom having a sister, and it's odd the sister would know any of those details.
