Yeah, that's the unrealistic part of the drama. Dr.Wandee in real life at the age of 26 should be working in a rural hospital somewhere in Thailand because all the med students are funded by the government and they have to work for a few more years to repay the scholarships they owed.

In Thailand, we go to med school like you go to college. You don't have to be undergrad to apply for a med school like in the west or your country. Wandee has known Ter for 8 years when he was a freshmen so Wandee might be around 26 because it takes 6 years to finish the medical degree here.
