This movie takes place in an alternate universe (where its storyline could be considered a prequel to the events of the TV series). In 1999, an enormous meteor crashed onto Earth carrying extra-terrestrial creatures. In order to counter this new threat, ZECT was formed. Seven years later, in the year 2006, power struggles in ZECT have caused a new group, known as Neo-ZECT. With three groups in existence (Neo-ZECT, ZECT and the Worms), Tendou has decided to sell his power to either Neo-ZECT or ZECT, but his intentions are unclear. ZECT reveal a plan to save humanity, but unknown to other is a plot to destroy man kind.

Wake up."

However, what he doesn't realise is that the two witnesses are the journalist who had interviewed him prior to his kidnapping, Asuka Midorikawa and her fiancé Katsuhiko Yano. Attacking the two along with Shocker soldier Spider, it is here that Hongo remembers his humanity, and has an epiphany with regards to what he should do.

The events pick up two years after the last film. Takeshi Hongo is now a high-school teacher and Hayato Ichimonji is just counting down the days before his cybernetic implants kill him. Our heroes are thrust into action when a bizarre series of murders occur; the calling card being the single "Platinum Smile" playing in the background. A further investigation by our heroes discovers that not only is its singer, Chiharu Kazami, dead, but the single itself is part of Shocker's latest nefarious scheme. The Double Riders soon find themselves battling a band of evil twins, led by two new Shocker Kaijin and Kamen Rider V3, who is none other than Chiharu's brother Shiro Kazami.
