My take, He suffers from the guilt of abandoning his brother in the water when he was younger. Letting them be separated and letting Sangtae be shipped off to a random facility could have been interpreted as some form of abandonment in his young mind. Truthfully a free mental health facility funded by the state wouldn’t have provided him with the care he needed and a private facility would’ve been to expensive. By taking his brother’s care into his hand’s only he is both appeasing his own guilty self and trying to provide him with as much care as he possibly can.
