He has a psychic disorder of a different nature, making it compatible with his helpful temperament.
Do you remember the scene where we see all his scars? His devotion is sickly, and he's wounded because of it, against his own good.
Let's not forget the pitch as well: he's a volunteer worker originally, not a professional. A friend of mine has become a psychologist, she studied psychology at university, and she told me that the only reason she was doing this in the beginning was to solve her own problems. And almost all the other students were the same way. The ML has to deal not only with his own problems, but also with those of his brother.

The uncertainty of whether a story ends well or badly should still be part of the suspense and excitement?! Maybe you felt like changing your habits and having the pleasure of living this experience, lol!
For my part, I prefer stories that have a sad ending, it's more moving.

I think the ML is as crazy as the FL. That's a quality of the story because it makes them two complementary characters, and irrevocably destined to fall in love.
A lot of clues are regularly left about this. Episode 1, his dissociation from pain. He stops the knife with his bare hand, bleeds, and shows no pain. Of course, it's classic in a sageuk, but not in an everyday story. If you watch the episodes again, you'll get a lot of other clues about it, like his best friend calling him a joker. Or simply, episode 3, he's ready to get run over by a car, not a trace of hesitation or movement, even when the car brakes at the last moment.

I dreamt I was friends with a little horse last night.
The horse wasn't white, she was light brown, with a fine golden mane.
The horse was sick, she was sneezing. When I asked the horse questions, she was able to answer yes or no, making a noise with her mouth.
I was sad for the horse, because in my dream I had to ask her to pull a little cart.
And I was trying to give her moral support.
It was a heartbreaking dream.

That's what the title of the drama suggests. ;-)
It happened in the Japanese drama Kimi Wa Petto.
But it would surprise me from a Korean drama. My first impression was that FL does not suffer from a psychic disorder of a psychiatric nature. So, she can evolve, recover psychologically, just like anyone else. Same with ML. The love between the two characters can lead to deep, sometimes painful, but life-saving introspection.

My first comment said that I felt like I was taking ---- in the face. Written too fast, not clever. When I wrote my comment again, I was annoyed, but now I see that it was useless to insert remarks related to that. By the way, I withdraw them.

I am not annoyed about the drama. Just disappointed, because I also liked the first two episodes.
In addition to the boring aspect of episode 3, there are two other things:
- The artificial side. Hard to describe, it's something I felt during Hotel Del Luna. After stopping the drama for a while, I tried to see the end, and everything seemed fake, superficial, phony. This is noticeable here in many scenes.
- The overplayed, grotesque side. Several scenes of the episode concerned, for example the end scenes. The car chase (with loud background music). The whole scene with the madman taking the microphone, especially the moment when he dances in slow motion with a song in the background.

I have lost confidence in drama because I think that this type of direction/narration is likely to be used systematically. It takes away my sense of credibility in the story. I much prefer a simpler and more classical drama in this case, such as a sageuk, where the actors have a slightly theatrical acting. Sophisticated directing suits me, but when I can take things seriously, or when the effects are not so clichéd.

Of course, other viewers may not be sensitive to all this, and may like the drama as it is. If that's the case with you, that's what I wish you. I'll try to read your comments to see if I'll come back to it later.
