Not killing everyone at night by mafia is not a plothole, but actual rule of the game. Mafia can kill only one person per night :)
Also, mafia indeed goes to sleep after they kill someone, honestly, in real card game, the night is divided between mafia and citizens with special powers.

Yes, they would expose themselves, but I do feel that mafia has very different mental stability? Aka they do not fear to kill as much as citizens, meaning if they form unity, they can eradicate others not only at night, but in day as well.
