Not killing everyone at night by mafia is not a plothole, but actual rule of the game. Mafia can kill only one person per night :)

Yes, they would expose themselves, but I do feel that mafia has very different mental stability? Aka they do not fear to kill as much as citizens, meaning if they form unity, they can eradicate others not only at night, but in day as well.

Mafia does not lose consciousness, they just go to sleep like normal people (at least, that's what should happen).
