What Kasumi is saying is this: Ayami is leaving because the thought of having another woman in Saitaro's life other than herself is unbearable for her to think. For her, Saitaro still looks up to Ayami as her mother. He would do anything for her, so leaving is her only choice to give Saitaro his freedom.

To anyone who doesn't understand the scene.
Short answer is this: Ayami leave because she feels that Saitaro doesn't need her anymore because he have Kouko-san (his wife) .
Long answer is this: Ayami still feels like she is not his mother, as confirmed on Saitaro's wedding at Fuusha. With Fuusha closing, she doesn't want to burden the newlywed with a financial burden of continuing Fuusha.
Throughout 115 to 118, there is a term called "Filial Piety" present in those episodes, I translated it as (Saitaro) "son's duty" to her parent (Ayami). When Ayami jokingly asked Saitaro to rebuild Moulin Rouge, Saitaro took it as his duty to make Ayami's final wish as her son. Though in Ayami's case she feels she is not qualified for it because once again, she is not his real mother.

I understand that episode 1 is kind of all over the place but that is how the Japanese sub was presented. Let me clarify episode 1 for you. This may contain spoiler.
At the beginning of the episode, Tsukamoto (Kasumi's manager) was informed that the director Kaibara has the flu. This gives Kasumi a day off from filming her fictional TV Series "Prosecutor Todoroki Tako".
She visits her hometown, where she is met with her mom. Along the episode, they did some catching up. Suddenly a stranger has visit their house. Kasumi's dad is dead in this series where he has an illegitimate son from another woman, Makoto, the stranger.
In the case of the phone call, she is calling if the director (Kaibara) is feeling better but the son of the director is the one that answered. Her reaction after the call has nothing to do with the call. It was frustration that she did not know anything about this Makoto or her dad having an illegitimate son and that her mother did not tell her about him.
I hope this clarifies episode 1 for you. Episode 2 is much better. I will be releasing the subtitle for free download on April 19 or 20 depending on where you are in the world. For now, it is on Patreon.
