The setting notes (along with Yukishima's conditional death) is the reason for the time loops. It would only have the power to do time loop if Mahoro and Yukishima meet on September 1, 2015, and Yukishima learning Mahoro's name. When Yukishima was explaining about the notebook, he first finds the notebook completely blank in the drawer with the last page saying that meeting Mahoro was the reason Yukishima can draw. When Yukishima found the notebook again, it is filled with notes for Revive Line, these notes were sent from the future to the past to force Yukishima to complete Revive Line, but universe always correct itself and Yukishima dies either by being killed or by his illness or the notebook being torn apart. His illness also is dependent on whether he used the notebook to make Revive Line. The final loop was success because Yukishima never found the notebook again and he never learned Mahoro's name. Hence, Mahoro went with Haruma pen name, so it doesn't cause time loop. Gorilla, Kirisato, Mahoro and Arashi, guide Yukishima to completing Revive Line by suggestions and influence.
In the final loop, there is no setting notes that Yukishima writes notes to, unlike in the original timeline where he wrote from start to finish, all those years the settings, characters, world building of Revive Line before being killed and had the chance to finish it.
To put it in order, original timeline Yukishima writes Revive Lines setting notes > got killed by Sayaka > desires of all the people wanting it to complete took residence in the setting notes > notebook possess the power to loop back time and was sent to the past > force Yukishima to finish Revive Line for time to move forward > completing it early causes universe to correct itself and loop back time again by having Yukishima dies from illness > Yukishima tearing the notebook for one final time loop > Mahoro avoids Yukishima and Arashi helping Yukishima finish Revive Line > Mahoro creates a manga to help Yukishima with Revive Line > Revive Line is complete > Yukishima learned about Mahoro's name > remembers everything from the time loops > kiss > series ends with the disappearance of the setting notes that was sent from the future as the condition for its existence is not met anymore.
Or in a simpler form, Yukishima must not meet and not know Mahoro's name before he can finish Revive Line. Remember when Gorilla recounts the original timeline, Yukishima was struggling with Revive Line, until he met by chance Mahoro who was working in the library. All they are doing in the final loop is remaking the original timeline, they never met again until he started serializing Revive Line, same with final loop, Mahoro only met Yukishima until he finished Revive Line, only then that the desire of the people for its completion was satisfied.

The last part happened inside an airport, hours away from Shunpei's flight to Germany. The orchestra abandoned their rehearsal in favor of sending off Shunpei. The concertmaster took over the conducting position for Harumi Philharmonic with Hibiki now as the first violinist and concertmaster for the orchestra. They will participate in the competition without Shunpei. It's an open-ended ending, where they left the future open (maybe for an SP) where Shunpei could return to Harumi Philharmonic and conduct for them.
Clara is the supposedly girlfriend of Taku in Switzerland. Sayako created her and told everyone at the house that Taku has a girlfriend named Clara. They all teases Taku about it.
