It is so relatable that Eun O is so confused. Just because Ji Hyeok doesn't tell Eun O what is going on with him doesn't mean he has to insist on treating her coldly. You would think he would be extra kind to her. He doesn't have to give her false hope of the future. He could, though, go on dates with her, and in the worst case, he would leave her with good memories of him. His conduct is at odds with someone who cares deeply for another. He has no problem showing extra kindness to his parents and his best friend.

Hopefully, we can get some progress this episode of them becoming a couple for real. Enough with the obstacles. Start to overcome them.

Maybe if the drama had opened with Ji Hyeok finding about his heart problem. His heart problem which he kept to himself as the reason why he didn't want to marry and rejected Eun Oh. The lame "I'm not good enough for you" excuse was a very weak reason for him to treat her the way he did during the first episodes.

The dumbass marry for money saga could have been left on the cutting room floor also. It didn't add anything to the story being told and was out of character for a man who went around and claimed he never wanted to marry even for love.

I am no fan of Jin Seok. But he gathered all the evidence and had a poker face the entire time before he acted. Really smooth work.

The couples should be:
Ji Hyeok and EunO
Seong Jae and Subin
Ji Wan and Yeong Ra

However, the writer has only created reasons why none of them can be together in romantic relationships. Interfering parents and now illness.
