@milka no, no. Absolutely everything you said is correct. I thought Na Yeon's heart could possibly save Do Hee, but it's more likely, Do Hee will just die and the fact we're spending so much time with Na Yeon will make more sense. In the beginning, I thought Na Yeon was a "false lead" like that Quentin Tarentino film where he kills of the whole cast midway through, and just puts a new one - or like Shut up Flower boy band, where Lee Min Ki is the "false lead" but he's killed 4 episodes in. But that is original and innovative storytelling. Maybe I was giving this kdrama too much credit.
I will be sad to see the real Do Hee go. But when.....wheeeeeeen will it happen?

Let's not dismiss the synopsis entirely, shall we? Do Hee is shown to be having heart problems. This is a guarantee trip to ER for open heart surgery (that's how these cliches usually work.) If Do Hee is going to ER with this problem, then guarantee Na Yeon will land in ER too. Na Yeon's good heart will save Do Hee. If Do Hee is going to be killed in a case of mistaken identity, then the "heart problems" gag is pointless. Trust me; they're planning to use it.

This drama certainly isn't a chore to watch. In fact, it's kind of perfect for people who have short attention spans. Because the episodes are 30mins, there's more cliffhangers than the usual drama. Plus, all the actors are good. EVERYBODY. No limp wood being carried here! :D
