I'v wondered about her from the start, and have been very surprised that no-one else seemed to suspect her. She is connected to everyone, has a lot of (scared) staff at her disposal, has a huge hosputal with a secret experimental area, where she could hide a lot of things, and she's definitely doing something unpleasant/illegal with all those cancer patients. I even wondered whether I had missed an important piece of information that cleared her.
As a fan of old mystery novels, I wonder whether the writer has cleverly used the old "hidden in plain sight" and "last person the reader expects" methods, distracting us with all the *bigger* characters.
There are a surprising number of people still putting forward the multiple personality disorder theory. I'm no mental health expert, but I'm pretty sure that makes no sense. First, he would need to be totally in control of switching between the personalities, aware of both of them, and never have accidentally switched during stressful moments. Seems highly unlikely. Also, Miho is a very good nurse and had been with him since he was quite young. I'm fairly sure she would have spotted symptoms. The writer is developing an intriguing plot, with realistic and interesting characters. I would actually be very disappointed if they have gone for Dissociative Identity Disorder as the answer. The only thing more disappointing would be an "it was all a dream" ending.
I need a big art book of every different setting used in this series, and all the beautiful costumes (not the plastic headdresses , but the rest of the costumes!). It would be an amazingly satisfying and relaxing thing to look at.

That would be a terribly clichéd ending. There was a time-travel Cdrama recently that I really enjoyed until the last 5 minutes, when it was all revealed to be a book the main character was writing and there was therefore no logical explanation for the story. Terribly frustrating, because the plot had been unusual and there were definitely ways to make it logical, but the writer took the easy way out.

Big Mouth is the main lead's nickname, so even if he's not Big Mouse, the title still describes the main character. I don't think he is. She's more realistic this way though. She's terrified, confused and doesn't know who to trust. In that situation it's more normal to react on instinct and blurt out demands/questions than it is to stay calm and think rationally. She has her intelligent moments, but I like that she's not shown as a brave superhero wife, saving the day, but as a normal flawed human, accidentally messing up plans while trying to do the right thing.

Both. If you go back and compare, you'll notice that as he gains emotions, not only his makeup gets softer, but many of his outfits get lighter in colour (there's the usual dark = evil/light = good symbolism in the outfits), and his expressions and actions become a bit less harsh. The costume/makeup change is easy to accomplish, but softening the expressions and behaviour a bit while still being a convincing Moon Supreme is much harder. Good acting!

I try never to judge an actor by just one of their roles, or a singer by just one of their songs. There are singers whose whole repertoire I hate , except for one song. Equally , there are actors that I only found convincing in one role (e.g., so far I can only stand watching Lee Dong Wook in Goblin.)

Yes, I agree. It's either a mythical title, which handily gets the blame for huge crimes and is obviously untraceable as "he" doesn't exist, OR it's the name of an organisation, so even if one member dies, "Big Mouse" keeps going.
Her acting is perfect. Her voice may irritate you if you don't like high-pitched or childlike voices. However, for this role, I think it fits her character. She is a naïve, inexperienced little fairy, and for that she sounds exactly right.
