I'm not really taking into consideration actual historical facts, since the very first scene of this drama suggests the story takes place in a country called joseon in a "different planet", therefore it makes it super clear that it isn't based on real facts (unlike the red sleeve which, as you mentioned, tells the story of an actual palace maid turned into a concubine). I also thought the logical outcome (had she really been just a commoner) was for her to become a concubine, but if I'm not mistaken both the minister who always advises the king and his personal/head eunuch mentioned that, by giving birth to an heir, she could be made queen in this story. The king himself even mentions a noble family "adopting" her so she can become his queen... I agree with you that all her problems in regards to status would be solved if she came foward with her true identity (especially bc of the queen dowager's prejudice against sorang being a "lowborn"). I had no idea about that haha that little piece of info makes me excited to see the reaction of the king's grandmother once she finds out about it, since all she cares about is status and she treated sorang pretty poorly so far
Havent seen anyone talking about it before but I think so rang might be pregnant... it was one of the ways for her to become queen (regardless of the king knowing about her true identity as a "noblewoman" or not) and they hinted at it a couple of times already so it could have been forshadowing... I suspect it especially after she told 2nd ML that she became the king's woman but they decided to keep it a secret (and a pregnancy isnt exactly something you can hide haha)
Is it possible to understand and have a good experience even if you havent watched the first season? I usually dont watch medical dramas for personal reasons but i really like both main leads so I'm honestly considering giving it a go...

Idk if you ever watched flower of evil but its the only married couple without cheating I can think of rn from the few I've watched and I loved their romance storyline!! Its 100% worthwhile!
